OCD (obsessive-compulsive disorder) has a lifetime prevalence of approximately 2.3% of adults in the United States and affects about 1.2% of adults annually, according to NIMH. This translates to around 3 million adults. There are also around 500,000 children and teens with OCD in the United States, as noted by the International OCD Foundation. At Iris Healing in Los Angeles, we offer comprehensive and personalized OCD treatment options for people with OCD, including TMS for OCD.

What Is TMS?

TMS, or transcranial magnetic stimulation, is a non-invasive form of therapy that targets specific regions of the brain responsible for neurotransmitter release and mood regulation. It uses magnetic waves to stimulate these brain regions. By stimulating these specific regions, many people notice a reduction in symptoms related to their OCD.

TMS for OCD

TMS for OCD involves determining which areas of the brain need to be targeted for symptom reduction. A device connected to the head and scalp delivers the magnetic waves to the targeted area. Next, one’s specific treatment protocols determine what frequency is used, how fast magnetic pulses are released, and the duration of the actual TMS treatment session.

Lastly, the TMS treatment session may involve exposure therapy just before TMS to trigger OCD obsessions. When the obsessions are triggered and TMS is applied, many people notice faster improvise and reduction in their symptoms.

Signs and Symptoms of OCD

OCD is considered a type of anxiety disorder because anxiety is often one of the most common signs and symptoms. Other signs and symptoms of OCD include:

  • Obsessions (unwanted, distressful, and intrusive thoughts)
  • Guilt
  • Doubt
  • Shame
  • Fear of losing control
  • Fear of self-harm or harming others
  • Excessive worry or fear
  • Compulsions (behaviors to relieve obsessions)
  • Repeatedly checking behind oneself
  • Excessive cleaning
  • Excessive hand washing
  • Organizing things in a specific way
  • Repeating certain behaviors in a specific order

How Effective Is TMS for OCD?

The effectiveness of TMS for OCD depends on the individual. Some people notice marked improvements in their OCD symptoms, while others may only notice slight improvements. TMS for OCD should be part of a comprehensive treatment plan that includes other evidence-based and holistic therapeutic modalities, such as:

What to Expect in a TMS Session

Each TMS session ranges between 20 to 40 minutes. Treatment sessions can be available with residential, PHP (partial hospitalization program), IOP (intensive outpatient program), and outpatient program treatment plans. The person will sit or lie down during the treatment session and should be in a comfortable position.

Before the TMS device is attached, the person will have a short, personalized exposure session to trigger their compulsions. Triggering the compulsions helps engage the brain’s circuitry related to their OCD. This makes it possible for TMS to target these areas and provide relief.

Next, the TMS device is attached to the top of the head and adjusted so that it targets the specific brain region being treated. The person can be given earplugs to block the clicking sounds the device makes as the magnetic waves cycle on and off. Some people prefer to listen to music during their treatments, which is perfectly acceptable.

During the actual treatment, some people notice a tapping or tingling sensation on their scalp in the region being treated. This is normal and usually subsides immediately following the treatment session. At the conclusion of the treatment, the device is removed and people are encouraged to remain sitting or lying down for about five minutes.

Can TMS Make OCD Worse?

There are some side effects of TMS for OCD that people should be aware of when considering this therapeutic modality. Some people will experience minor scalp pain and discomfort following a treatment session. This usually subsides within an hour or two of the session. Other side effects could include:
  • Headaches
  • Facial muscle spasms and tingling sessions
  • Dizziness
  • Lightheadedness
Two of the more rare side effects are seizures and the worsening of symptoms. Symptoms could worsen in certain situations, especially those related to anxiety and depression. Additionally, when the individual is struggling with a co-occurring disorder, like alcohol addiction and OCD, they may notice a worsening of symptoms with TMS. It is essential to remember both of these are very rare side effects that normally do not occur in the majority of people receiving TMS for OCD.
